• Is the trust level low in the organization? If so, why?
• Do problems just somehow resolve on their own only to crop back up later?
• Does leadership think methodologies/tools are a magic bullet? 
• Is firefighting considered a badge of honor within the current culture?
• You cannot solve problems you cannot see. Does your organization have blind spots and unsurfaced problems? If so, why?
• The best methods/tools in the world do not matter unless problems are surfaced.  What are the reasons problems might not be surfaced?
• Problems will not be surfaced by employees unless trust is present. If this is true, why is trust broken or damaged?
